If you've spent any time on wellness TikTok lately, you've probably seen it: someone clipping a cheap electrode onto their ear or neck, claiming it's "the same thing" as a purpose-built vagus nerve device — just a fraction of the price.

TENS stands for transcutaneous electrical nerve stimulation, and EMS units work on a similar principle. Both were designed for one main job: temporary pain relief. They send electrical pulses through the skin to interrupt pain signals or trigger muscle contractions — think sore lower back, knee pain, or physical therapy for a strained muscle. That's a real, well-established use case, and it's what these devices were engineered and tested for.
What they weren't designed for is neuromodulation — specifically targeting the vagus nerve to influence the parasympathetic nervous system. It's a different goal that requires different tools.
This is the part most viral videos skip entirely. Vagus nerve stimulation research doesn't use "electricity in general" — it uses fairly specific electrical parameters. Studies on transcutaneous vagus nerve stimulation (tVNS) commonly cite a frequency range around 20–50 Hz, with roughly 25 Hz showing up repeatedly as a reference point, paired with a pulse width in the 200–500 microsecond range.
Most consumer TENS units don't make these settings easy to find, let alone dial in precisely — the display usually just shows an intensity bar, not the underlying frequency or pulse width. So even if someone wanted to replicate a research protocol on a generic device, they'd largely be guessing.
This is where a lot of viral TikTok hacks fall apart on closer inspection.
On the ear: research points to the cymba concha (a small area of cartilage near the top of the ear canal) as having the highest concentration of vagus nerve innervation. Many cheap devices clip onto the tragus instead — the small flap of cartilage in front of the ear canal — which studies suggest gives partial stimulation at best, not equivalent access to the nerve.
On the neck: the relevant access points sit at a very specific depth and location on either side of the neck, in an area close to the carotid sheath. That's not a minor detail — the carotid sinus in that same region plays a role in regulating heart rate and blood pressure through the baroreceptor reflex. Stimulating too broadly or too close to the wrong structure isn't just "less effective," it's a different risk profile entirely.
This isn't just theoretical caution. A systematic review published in the journal Brain Stimulation (Redgrave et al., 2018) looked specifically at the safety and tolerability of transcutaneous vagus nerve stimulation, and adverse effects have been documented with generic or unregulated devices used for this purpose — including unintended muscle contractions, increased anxiety, dizziness, and nerve pain. In the neck specifically, improperly targeted stimulation near the carotid area has been associated with heart rate and blood pressure fluctuations.
None of this means electrical stimulation itself is dangerous — it's widely used and well tolerated in properly designed devices and protocols. It means that where and how the current is delivered isn't a detail you can safely improvise with a device built for a completely different purpose.
Mostly because it's cheap, it's dramatic on camera, and "life hack" framing travels well on social media. None of that is evidence that a device designed for muscle pain relief is doing the same job as a device engineered and positioned specifically for vagus nerve access. Some of it might produce a sensation — but a sensation isn't the same as targeted, validated stimulation of the nerve you're trying to reach.
If a device wasn't designed, tested, and positioned specifically for vagus nerve stimulation, it's reasonable to be skeptical that clipping it onto your ear or neck replicates what the research is actually describing. Purpose-built vagus nerve devices — including wearables in this newer, non-invasive category — are designed around the frequency ranges, pulse parameters, and anatomical placement that the research points to, rather than repurposing a tool built for something else entirely.
